Case Management
Case Management services are a vital step in our process in order to provide successful outcomes for our youth. Our services range from resource linking to the community, to helping discover and maintain positive connections in client’s lives. Many youth come into our programs with very little connections to positive influences as well as community resources.
It is important that our case managers assess and understand each youth’s individual needs in order to provide the correct resources. Fostering positive relationships between adolescents and adults builds trust and rapport with adolescents through consistent support and modeling healthy relationships. Building permanent connections has a profound effect on a sense of belonging and increased opportunities for youth allowing for stable and loving relationships.
We believe that every youth should have a positive connection in their life, and some of these connections become possibilities for future placement!
